We are currently recruiting for this excellent temporary part time opportunity in Watford 22.5hours per week.
This is paying £11.50 per hour LTD
Normal working hours will be 0900 – 1700 three days per week. Will be required to work occasional weekends and bank holidays 1000- 1400 to cover the PCRC 7 day a week service. Will be required to cover for HCT Specialist Palliative Care Team as required. Administrator for the Communities Services Team – including coordination of bank staff for planned off-duties
Expected to be flexible in hours/days between Our clients Care Community Services, HCT Specialist Palliative Care Team and Community Services and the PCRC to meet the needs of the services.
Community Services duties and responsibilities under the direction of the Community Services Manager
Provide high quality administrative support on a day-to-day basis to the Community Services Team, which includes Hospice at Home (HAH), Herts Neighbours (HN) and the Community Services Manager/Team Lead.
To receive telephone calls to the Community Services Team and deal with them appropriately, maintaining a sensitive and professional approach at all times. Providing non-clinical advice, information and guidance as appropriate.
In-put Herts Neighbours diary sheets on a regular basis.
To manage and continuously improve all aspects of the administration process and procedures for the Community Services Team.
To liaise with GP surgeries to obtain dates of all palliative care meetings/GSF’s, and communicate these to the HAH team and Team Manager, and ensuring that all dates are recorded in the electronic diary.
Coordinate and ensure processes are in place to support new outpatient assessment and review clinics.
Support patients attending out-patient appointments, showing them to the waiting area.
Produce high quality documents from hand written notes and/or audio using word processing and other computer software as required.
Take minutes of formal and team meetings as required; ensuring appropriate follow up action is taken.
Maintain office systems and procedures, for the Community Services Team, which includes collating patient information packs.
To support the ordering, provision and storage of adequate supplies of stationery for the team, ensuring that everything is always in-stock and available.
To up-date the internal intranet for Community Services.
Provide statistics for data collection reporting purposes within agreed timescales as requested.
Be the SystmOne champion for Community Services, this includes attending meetings and being responsible for updating staff within the team and providing training for all new team members during their induction phase.
Support the Community Services Manager in scheduling all staff meetings, 1to1’s and appraisals, including room bookings.
West Herts Palliative Care Referrals Centre Duties and Responsibilities under the direction of the Community Services Manager:

To make phone calls to referrers in order to gain any missing information from referral forms in line with the referrals process

To set up new patients on all relevant SystmOne modules, including referral and acceptance to a particular service. Following the referrals process in a timely fashion. |
|
To accurately record all referrals received on the appropriate data base/log.
|
|
To maintain all records to a high standard, including updating waiting lists and referral outcomes.
|
|
To maintain all records including monthly log of referrals including reason for referral, phases and status, reason for acceptance/non acceptance, date of admission or when first seen by service and maintain appropriate filing.

|
To receive incoming calls and deal appropriately with them. Maintain a sensitive approach at all times.
|
|
To liaise with 4 providers on a daily basis to gain information regarding capacity and demand of individual services workload / bed state.
To be responsible for ensuring all administrators are working consistently.
To be responsible for the PCRC 7 day per week rota, ensuring that all shifts are covered at all times.
Under the supervision of the Community Services Manager provide reports and data as needed.
To lead on, including delegating, any admin actions and tasks identified. |
